Statement

It is recognized that at times it is necessary for students to take medication during the school day. In order to assure safe administration of prescription and over-the-counter medication, the following procedures will be observed.

Rules

1. No medication will be administered without the prior written consent of the parent or legal guardian.
2. Non-prescription medications will be handled in the same manner as prescription medications.
3. All prescription medications must be received in the original container, properly labeled with: Name of Student; Directions for Giving Medicine (Amount to be given and When it is to be given; Prescribing healthcare professionals name; Name of the medicine; Route of Administration (oral, topical, inhaled, injection, etc)
4. The completed and signed Medication Form must be brought or faxed to the school before any medicine is given.
5. Any change in medication (such as time or amount to be given) will require a new order by the prescribing healthcare professional and a new completed and signed providing the written consent of the parent or legal guardian.
6. Administration of all medications will be recorded in Infinite Campus.
7. No student shall be allowed to keep or administer medicine at school unless the proper form is completed. Any medicine to be administered to a student shall be brought to school health office where it will be kept in a secure place. All medications must be stored in a locked cabinet or container.
8. A student with a chronic disease or medical condition may possess and self-administer medication for the chronic disease or medical condition. The following must be completed.
9. The student’s parent has filed an authorization for the student to possess and self-administer the medication. Such authorization is kept on file in the Health Office.
10. The authorization must include a written statement by the physician advising that: That student has an acute or chronic disease or medical condition for which the physician has prescribed medication; The student has been instructed in how to self-administer the medication; The nature of the disease or medical condition requires emergency administration of the medication; The authorization and statement described in this provision must be filed annually.
11. At any time the parent may revoke permission of administer medications. This revocation must be submitted in writing to the school
12. No medication will be sent home with a student once such medication is delivered to the school corporation, except medication possessed by a student for self-administration.
13. Medications will only be released to the parent or legal guardian who provided the initial consent for administration of such medicine. Parents or legal guardians may designate in writing the release of specified medications to an individual who is at least 18 years old.
14. Medications not retrieved by the parent or legal guardian who provided the initial consent for administration of each medication will be disposed of on the last student day of the current school year.
